Payment and Cancellation Policy
A completed registration form and credit card information is requested and kept in a confidential file with your medical record. Co payments are collected on the day of service. Your insurance company will be billed after your visit. If you have a balance you will receive a bill from our office and you may pay with your credit card or check.
Please be advised that twenty-four hour notice on a business day is required for cancellations. It is important that you arrive for your appointment on time. Late arrivals may need to be re scheduled. New patients require a full hour of the doctor’s time. Follow up appointments are scheduled for thirty minutes. A $200 fee is charged for late cancellation or failure to cancel, since it is our policy not to schedule more than one patient in an appointment slot.
